INDIVIDUAL	Patient Fees
Age < 30	$100/month ($1,200/yr)**
Age 30–44*	$185/month ($2,220/yr)**
Age 45+*	$250/month ($3,000/yr)**

* Fee reduced 15% for spouse or domestic partner of members who are aged 30+.
** Payments can be made on an annual, quarterly, or monthly basis. 3% transaction charge for credit or debit card payment. No transaction charge for payment by ACH/bank payment.
